Diffuse large B-cell lymphoma (DLBCL) is the most common aggressive non-Hodgkin lymphoma, representing approximately 25–30% of all lymphomas. Standard first-line therapy is R-CHOP (rituximab, cyclophosphamide, doxorubicin, vincristine, prednisone) in fit patients. Cell-of-origin (COO) classification — germinal center B-cell (GCB) versus activated B-cell (ABC) subtype — and MYC/BCL2/BCL6 double-hit or triple-hit status influence prognosis and investigational regimen selection. CAR-T cell therapies (axicabtagene, lisocabtagene, tisagenlecleucel) and bispecific antibodies (epcoritamab, glofitamab, mosunetuzumab) have transformed the relapsed/refractory landscape.
