Metastatic colorectal cancer (mCRC) is the fourth most commonly diagnosed cancer and the second leading cause of cancer death in the United States. Extended RAS (KRAS/NRAS exons 2–4) and BRAF V600E mutational profiling is mandatory before initiating first-line therapy, as RAS-wild-type / left-sided tumors benefit from anti-EGFR antibodies (cetuximab, panitumumab) while BRAF V600E-mutant disease is treated with encorafenib + cetuximab in second-line. MSI-H/dMMR tumors — approximately 3–5% of mCRC — respond to immune checkpoint inhibitors and may receive pembrolizumab as first-line therapy.
