Non-small cell lung cancer (NSCLC) accounts for approximately 85% of all lung cancers and encompasses three major histological subtypes: adenocarcinoma, squamous cell carcinoma, and large cell carcinoma. Comprehensive molecular profiling — including EGFR, ALK, ROS1, KRAS G12C, MET exon 14, BRAF V600E, RET, NTRK, and PD-L1 — is mandatory before initiating systemic therapy, as each actionable alteration has approved targeted agents with superior outcomes versus chemotherapy. Stage-IV non-squamous NSCLC without a driver mutation is treated with pembrolizumab-based chemoimmunotherapy; high PD-L1 expressors (TPS ≥50%) may receive pembrolizumab monotherapy.
