Epithelial ovarian cancer is the most lethal gynecologic malignancy, with approximately 80% of patients presenting at advanced stage (III–IV) due to the lack of effective early screening. Standard first-line treatment consists of debulking surgery followed by carboplatin + paclitaxel ± bevacizumab. PARP inhibitor maintenance therapy (olaparib, niraparib, rucaparib) after platinum response has significantly extended progression-free survival, particularly in patients with BRCA1/2 mutations or high HRD scores. BRCA1/2 germline testing is now standard for all patients with non-mucinous epithelial ovarian cancer at diagnosis.
