How does Cetuximab work (mechanism of action)?

Chimeric (mouse/human) IgG1 monoclonal antibody targeting the extracellular domain of the epidermal growth factor receptor (EGFR/HER1). Competitively inhibits EGF and TGF-α binding to EGFR, blocking ligand-induced phosphorylation and downstream RAS/RAF/MEK/ERK and PI3K/AKT signaling. As an IgG1, cetuximab also mediates antibody-dependent cellular cytotoxicity (ADCC). Importantly, any activating mutation in RAS (KRAS or NRAS, exons 2–4) renders cells independent of EGFR signaling, abolishing cetuximab benefit in colorectal cancer.

Does Cetuximab have a black box warning?

Yes. Cetuximab carries FDA black box warning(s) regarding: 1. Severe infusion reactions: fatal anaphylaxis reported in ~3% (higher in southeast US — attributed to pre-existing IgE antibodies against galactose-alpha-1,3-galactose, a cetuximab glycan epitope). Reactions are most common during the first infusion. Epinephrine, corticosteroids, and resuscitation equipment must be available. 2. Cardiopulmonary arrest: sudden death during cetuximab + radiation therapy has occurred; monitor electrolytes (especially magnesium and potassium) and maintain normal levels.
