Mechanism of Action

Human IgG1κ monoclonal antibody targeting CD38, a type II transmembrane glycoprotein highly expressed on multiple myeloma cells (and more modestly on normal plasma cells and some hematopoietic progenitors). Daratumumab eliminates myeloma cells via multiple mechanisms: complement-dependent cytotoxicity (CDC), antibody-dependent cellular cytotoxicity (ADCC), antibody-dependent cellular phagocytosis (ADCP), and induction of apoptosis via Fc-mediated crosslinking. Clinical Pearl

Daratumumab causes a positive indirect antiglobulin test (pan-agglutination) because it binds CD38 on donor red blood cells used in compatibility testing. This can persist for up to 6 months after the last dose. Notify the blood bank before starting therapy and ensure baseline blood type and screen are performed; use dithiothreitol (DTT) treatment of reagent RBCs or genotyping to resolve alloantibody workups.
