Mechanism of Action

Recombinant human erythropoietin (rHuEPO) with the identical 165 amino acid sequence to endogenous erythropoietin, produced in Chinese hamster ovary (CHO) cells. Binds the erythropoietin receptor (EPOR) on erythroid progenitors in bone marrow, activating the same JAK2/STAT5, RAS/MAPK, and PI3K/AKT pathways as darbepoetin alfa. Shorter half-life than darbepoetin (~8.5 h IV, ~24 h SC) — requires 3× per week dosing vs once-weekly or once-every-3-weeks for darbepoetin. Epoetin alfa and darbepoetin alfa have equivalent clinical efficacy for chemotherapy-induced anemia; the key practical difference is dosing frequency — epoetin requires 3× weekly injections vs darbepoetin's once-weekly or once-every-3-weeks schedule, making darbepoetin generally preferred in oncology for convenience. Both carry identical black box warnings and APPRISE REMS requirements. Retacrit is the FDA-approved biosimilar of epoetin alfa and is designated interchangeable.
