Mechanism of Action
Fludarabine is a purine nucleoside analog that is phosphorylated intracellularly to its active triphosphate form (F-ara-ATP). F-ara-ATP competitively inhibits DNA polymerase alpha and epsilon, ribonucleotide reductase, and DNA primase, thereby blocking DNA synthesis and repair. Incorporation of F-ara-ATP into DNA leads to chain termination and induction of apoptosis. It also inhibits RNA synthesis by incorporating into RNA strands.
FDA Indications
- B-cell chronic lymphocytic leukemia (CLL), second-line after at least one prior alkylating agent regimen (FDA approved 1991, pivotal MDACC single-arm trials)

- Myelosuppression (neutropenia, thrombocytopenia, anemia)
- Fatigue and weakness
- Nausea and vomiting
- Fever and chills
- Infection (bacterial, viral, opportunistic)
- Peripheral edema
- Cough and dyspnea
- Diarrhea
- Stomatitis/mucositis
- Rash
Clinical Pearl
Fludarabine causes profound and prolonged T-cell immunosuppression (CD4 counts can remain depressed for >1 year), necessitating PCP prophylaxis (trimethoprim-sulfamethoxazole), antiviral prophylaxis (acyclovir), and use of irradiated/CMV-safe blood products to prevent transfusion-associated GVHD.
