Mechanism of Action

Thalidomide analog (immunomodulatory imide drug, IMiD). Binds cereblon (CRBN), a component of the CRL4-CRBN E3 ubiquitin ligase complex. CRBN binding redirects the ubiquitin ligase to degrade transcription factors Ikaros (IKZF1) and Aiolos (IKZF3), which are essential for myeloma cell survival and pro-survival cytokine production. Also inhibits TNF-α, stimulates T-cell and NK-cell activity, and has anti-angiogenic properties.

VTE prophylaxis is mandatory with lenalidomide — aspirin 81–325 mg/day for low-risk patients; LMWH for high-risk (prior VTE, surgery, immobilization, obesity). The REMS program requires enrollment of both patients and prescribers — failure to comply is a prescribing violation.
