Mechanism of Action

Methylprednisolone is an intermediate-acting synthetic glucocorticoid with roughly 5 times the anti-inflammatory potency of hydrocortisone and negligible mineralocorticoid activity. It binds the cytoplasmic glucocorticoid receptor (GR-α); the activated ligand-receptor complex translocates to the nucleus to modulate transcription via glucocorticoid response elements, suppressing NF-κB-driven pro-inflammatory cytokine synthesis (IL-1, IL-2, IL-6, TNF-α) and inducing apoptosis in lymphoid cells through upregulation of pro-apoptotic mediators and downregulation of BCL-2. In ESHAP, methylprednisolone 500 mg IV daily × 4–5 days is the "S" (formerly written as Solu-Medrol) and provides high-dose glucocorticoid lymphocytolysis alongside etoposide, cytarabine, and cisplatin. Distinguish the salts: Solu-Medrol (succinate) is the IV formulation, whereas Depo-Medrol (acetate) is a depot suspension that must never be given intravenously.
