Mechanism of Action

Mitomycin is a bioreductive alkylating agent derived from Streptomyces caespitosus. It is a prodrug that undergoes enzymatic reductive activation by intracellular reductases (primarily DT-diaphorase and cytochrome P450 reductase) under hypoxic conditions to generate highly reactive alkylating intermediates, including semiquinone radicals and aziridinium ions. These activated species form covalent interstrand and intrastrand DNA cross-links, preferentially at N2-guanine positions in CpG sequences, irreversibly inhibiting DNA replication and transcription and triggering apoptosis. In the definitive chemoradiation treatment of localized anal canal squamous cell carcinoma (the Nigro regimen), mitomycin C is an irreplaceable component — multiple randomized trials (RTOG 98-11, ACT II) confirmed that substituting cisplatin for mitomycin C resulted in inferior locoregional control and worse colostomy-free survival.
