Mechanism of Action
Mitomycin is a bioreductive alkylating agent derived from Streptomyces caespitosus. It is a prodrug that undergoes enzymatic reductive activation by intracellular reductases (primarily DT-diaphorase and cytochrome P450 reductase) under hypoxic conditions to generate highly reactive alkylating intermediates, including semiquinone radicals and aziridinium ions. These activated species form covalent interstrand and intrastrand DNA cross-links, preferentially at N2-guanine positions in CpG sequences, irreversibly inhibiting DNA replication and transcription and triggering apoptosis. FDA Indications
- Disseminated adenocarcinoma of the stomach and pancreas: palliative treatment in combination with other chemotherapy agents (IV formulation)
- Anal canal carcinoma: in combination with 5-fluorouracil and radiation therapy (Nigro chemoradiation regimen; IV formulation)
- Low-grade upper tract urothelial carcinoma (LGUTUC): endoscopic treatment of adult patients following complete endoscopic resection (Jelmyto — mitomycin gel, pyelocalyceal instillation)
- Non-muscle-invasive bladder cancer (NMIBC): intravesical instillation as adjunct to transurethral resection (TUR) in select patients (Mitosol / generic intravesical formulation)
Common Side Effects
- Myelosuppression: thrombocytopenia and leukopenia (≥64%; delayed, nadir weeks 4–6)
- Nausea and vomiting (≥14%; moderately emetogenic IV)
- Anorexia and fatigue (≥14%)
- Stomatitis / mucositis (≥14%; IV administration)
- Alopecia (≥14%; IV administration)
- Fever (≥14%)
- Bladder irritation (dysuria, urinary frequency, urgency, hematuria — with intravesical use)
- Rash and skin toxicity (≥10%)
- Elevated serum creatinine (≥2%)
Clinical Pearl
In the definitive chemoradiation treatment of localized anal canal squamous cell carcinoma (the Nigro regimen), mitomycin C is an irreplaceable component — multiple randomized trials (RTOG 98-11, ACT II) confirmed that substituting cisplatin for mitomycin C resulted in inferior locoregional control and worse colostomy-free survival.
