Mechanism of Action
Mogamulizumab is a humanized, defucosylated anti-CCR4 (CC chemokine receptor 4) IgG1 monoclonal antibody. CCR4 is a G protein-coupled chemokine receptor overexpressed on malignant T cells in mycosis fungoides (MF) and Sézary syndrome (SS), as well as on regulatory T cells (Tregs). Defucosylation of the Fc region markedly enhances binding affinity for FcγRIIIa on NK cells and macrophages, substantially augmenting antibody-dependent cellular cytotoxicity (ADCC) and antibody-dependent cellular phagocytosis (ADCP) compared with fucosylated antibodies. Clinical Pearl
Mogamulizumab's most critical safety consideration prior to initiation is the risk of severe, potentially fatal GvHD if allogeneic stem cell transplantation is performed within approximately 50 days of the last dose — CCR4+ Treg depletion impairs GvH tolerance, dramatically increasing non-relapse mortality.
