Mechanism of Action
Paclitaxel bound to albumin nanoparticles (130-nm particles). Albumin carrier allows Cremophor-free formulation, eliminating Cremophor-associated hypersensitivity reactions. Albumin-binding protein (SPARC/gp60) pathway facilitates preferential transcytosis into tumor tissue. Same microtubule-stabilizing mechanism as paclitaxel.
FDA Indications
- Metastatic breast cancer — after failure of combination chemotherapy or relapse within 6 months of adjuvant therapy
- Locally advanced or metastatic NSCLC — first-line with carboplatin (non-curative surgery/radiation candidates)
- Metastatic pancreatic cancer — first-line with gemcitabine
Common Side Effects
- Peripheral neuropathy (similar or less than Taxol)
- Myelosuppression
- Alopecia
- Fatigue
- Nausea
- Myalgias
Clinical Pearl
NOT interchangeable with Taxol — different doses, schedules, and formulation. Nab-paclitaxel does not require premedication and avoids Cremophor-mediated hypersensitivity. The albumin-binding mechanism may provide superior tumor penetration. In pancreatic cancer, Abraxane + gemcitabine (MPACT trial) demonstrated a landmark survival benefit, establishing this as a standard first-line option.
