Mechanism of Action
Necitumumab is a recombinant human IgG1 monoclonal antibody that binds to the human epidermal growth factor receptor (EGFR) and blocks the binding of EGFR to its ligands. This binding inhibits receptor phosphorylation and downstream signaling pathways, including MAPK and PI3K. Additionally, it induces antibody-dependent cellular cytotoxicity (ADCC) in EGFR-expressing tumor cells. In non-small cell lung cancer (NSCLC) models, necitumumab inhibits tumor growth and increases apoptosis.
FDA Indications
- In combination with gemcitabine and cisplatin, for first-line treatment of patients with metastatic squamous non-small cell lung cancer.

Clinical Pearl
Necitumumab is specifically indicated for squamous NSCLC; it failed to show benefit in non-squamous histology in the INSPIRE trial. The risk of sudden death is a unique and serious concern compared to other EGFR inhibitors, likely related to profound electrolyte wasting. Clinicians must be vigilant in aggressive electrolyte replacement, often requiring IV supplementation even in asymptomatic patients.
