Mechanism of Action

Neratinib is an orally bioavailable, irreversible (covalent) pan-HER tyrosine kinase inhibitor that forms permanent covalent bonds with specific cysteine residues in the ATP-binding clefts of HER family kinase domains: Cys-805 in HER2 (ErbB2), Cys-773 in EGFR (HER1/ErbB1), and Cys-803 in HER4 (ErbB4). The ExteNET trial (n=2,840 HER2+ early breast cancer patients after completing trastuzumab-based adjuvant therapy) demonstrated that 1 year of neratinib significantly reduced invasive disease-free survival events vs placebo (iDFS HR 0.73; 5-year iDFS 90.2% vs 87.7%), with the greatest absolute benefit in the HR+/HER2+ subgroup (HR 0.60; 5-year iDFS 91.2% vs 86.8%).
