Mechanism of Action
Neratinib is an orally bioavailable, irreversible (covalent) pan-HER tyrosine kinase inhibitor that forms permanent covalent bonds with specific cysteine residues in the ATP-binding clefts of HER family kinase domains: Cys-805 in HER2 (ErbB2), Cys-773 in EGFR (HER1/ErbB1), and Cys-803 in HER4 (ErbB4). FDA Indications
- Extended adjuvant treatment of adult patients with early-stage HER2-overexpressed/amplified breast cancer, following adjuvant trastuzumab-based therapy (FDA approved July 2017; ExteNET trial)
- Metastatic HER2-positive breast cancer — in combination with capecitabine, in adults who have received ≥2 prior anti-HER2-based regimens in the metastatic setting (FDA approved February 2020; NALA trial)

- Diarrhea (>95% overall; Grade 3 in ~17% with prophylaxis, up to 40% without)
- Nausea (43%)
- Abdominal pain and cramping
- Vomiting (26%)
- Fatigue / asthenia (27%)
- Decreased appetite / anorexia (18%)
- Rash / acneiform dermatitis (18%; EGFR-mediated)
- Dry skin
- Stomatitis / mucositis

Clinical Pearl
The ExteNET trial (n=2,840 HER2+ early breast cancer patients after completing trastuzumab-based adjuvant therapy) demonstrated that 1 year of neratinib significantly reduced invasive disease-free survival events vs placebo (iDFS HR 0.73; 5-year iDFS 90.2% vs 87.7%), with the greatest absolute benefit in the HR+/HER2+ subgroup (HR 0.60; 5-year iDFS 91.2% vs 86.8%).
