Mechanism of Action
Obecabtagene autoleucel (obe-cel) is an autologous CD19-directed CAR-T cell therapy with a proprietary "fast-off" CD19-binding domain — a modified version of the FMC63 scFv with reduced target affinity compared to conventional CD19 CAR-Ts (tisagenlecleucel, axicabtagene). This lower-affinity kinetic profile allows the CAR-T cells to engage CD19-positive tumor cells, mediate cytotoxic killing, and then rapidly disengage — mimicking the serial killing behavior of natural cytotoxic T cells. - Relapsed or refractory B-cell precursor acute lymphoblastic leukemia (ALL) — adults (FDA approved November 8, 2024; expanded February 26, 2026)

Clinical Pearl
Obe-cel's defining clinical advantage is its dramatically reduced CRS and ICANS toxicity profile compared to earlier CD19 CAR-Ts in adult B-ALL — a historically high-toxicity population. In the pivotal FELIX trial, Grade ≥3 CRS occurred in only 3% of patients (vs. 20–26% with tisagenlecleucel), while maintaining high efficacy: CR/CRi rate 72%, MRD negativity 97%.
