Mechanism of Action

Second-generation nitrogen-containing bisphosphonate. Binds with high affinity to hydroxyapatite crystals at sites of active bone remodeling, where it is preferentially deposited. As osteoclasts resorb the mineralized matrix, pamidronate is internalized and inhibits farnesyl pyrophosphate synthase (FPPS) in the mevalonate pathway, depleting the isoprenoid lipids (geranylgeranyl pyrophosphate) required for prenylation of the small GTPases (Ras, Rho, Rac) that maintain osteoclast cytoskeletal architecture, ruffled-border formation, and survival. Pamidronate and zoledronic acid share the same mechanism, but the head-to-head ZOMETA trial showed zoledronic acid achieves faster and more complete normalization of hypercalcemia with a 15-minute infusion versus pamidronate's 2–4+ hour infusion — so zoledronic acid is generally preferred when available.
