Mechanism of Action
Pertuzumab is a recombinant humanized IgG1κ monoclonal antibody that targets the extracellular dimerization domain (subdomain II) of HER2/ErbB2 — a distinct, non-overlapping epitope from trastuzumab, which binds subdomain IV. By occupying the HER2 dimerization arm, pertuzumab sterically blocks HER2 from forming heterodimers with other HER family receptors, most critically HER3 (ErbB3) and HER1 (EGFR), preventing ligand-driven receptor activation and downstream signaling through PI3K/AKT/mTOR and RAS/MAPK pathways. FDA Indications
- Neoadjuvant treatment of adults with HER2-positive, locally advanced, inflammatory, or early-stage breast cancer (tumor >2 cm or node-positive) — in combination with trastuzumab and chemotherapy as part of a complete treatment regimen for early breast cancer
- Adjuvant treatment of adults with HER2-positive early breast cancer at high risk of recurrence — in combination with trastuzumab and chemotherapy
- First-line treatment of adults with HER2-positive metastatic breast cancer — in combination with trastuzumab and docetaxel (CLEOPATRA trial)

The CLEOPATRA trial (n=808, first-line HER2+ MBC) established pertuzumab + trastuzumab + docetaxel as the global standard of care: median OS was 56.5 months in the pertuzumab arm vs 40.8 months in the placebo arm (HR 0.68), representing one of the largest OS improvements ever achieved by adding a single agent to a backbone regimen in metastatic breast cancer.
