Mechanism of Action

Mechanistically complementary two-drug combination for advanced pancreatic neuroendocrine tumors (pNETs). Streptozocin is a nitrosourea-type alkylating agent that exploits GLUT2-mediated uptake into pancreatic islet-derived cells, alkylating DNA at O6-guanine and causing strand breaks. Doxorubicin is an anthracycline that intercalates into DNA, inhibits topoisomerase II, and generates reactive oxygen species — mechanisms entirely distinct from streptozocin alkylation. The Moertel ECOG study (NEJM 1992) established STZ + doxorubicin as a standard regimen for advanced pNETs, demonstrating a 69% biochemical response rate and 36-week median time to progression — meaningfully superior to STZ + 5-FU (45% response) and streptozocin monotherapy (36%) in that dataset.
