Mechanism of Action
Mechanistically complementary two-drug combination for advanced pancreatic neuroendocrine tumors (pNETs). Streptozocin is a nitrosourea-type alkylating agent that exploits GLUT2-mediated uptake into pancreatic islet-derived cells, alkylating DNA at O6-guanine and causing strand breaks. Doxorubicin is an anthracycline that intercalates into DNA, inhibits topoisomerase II, and generates reactive oxygen species — mechanisms entirely distinct from streptozocin alkylation. FDA Indications
- Metastatic islet cell carcinoma of the pancreas — streptozocin is FDA-approved for use in combination with doxorubicin for this indication (approved 1982, based on the Moertel/ECOG data)
Common Side Effects
- Severe nausea and vomiting (>90% with streptozocin; highly emetogenic; combination worsens emetic burden)
- Myelosuppression — neutropenia, thrombocytopenia, anemia (primarily from doxorubicin; nadir days 10–14)
- Renal tubular dysfunction — glucosuria, proteinuria, hypophosphatemia (streptozocin)
- Alopecia (complete; doxorubicin)
- Mucositis (doxorubicin)
- Fatigue and anorexia
- Diarrhea
- Elevated LFTs (streptozocin)
- Red/orange urine discoloration (doxorubicin — expected; reassure patient)
Clinical Pearl
The Moertel ECOG study (NEJM 1992) established STZ + doxorubicin as a standard regimen for advanced pNETs, demonstrating a 69% biochemical response rate and 36-week median time to progression — meaningfully superior to STZ + 5-FU (45% response) and streptozocin monotherapy (36%) in that dataset.
