Mechanism of Action

Oral, potent, CNS-penetrant, next-generation ROS1 tyrosine kinase inhibitor. Taletrectinib inhibits ROS1 fusion oncoproteins, including key resistance mutations such as ROS1 G2032R that limit earlier ROS1 TKIs, blocking downstream proliferative and survival signaling in ROS1-rearranged tumors. Its brain penetration produces intracranial responses in patients with CNS metastases, which are common in ROS1-positive NSCLC.

FDA Indications

• Locally advanced or metastatic ROS1-positive non-small cell lung cancer (NSCLC) in adults (FDA approved June 11, 2025; TRUST-I and TRUST-II: confirmed ORR 85-90% in TKI-naive and 52-62% in TKI-pretreated patients)

Clinical Pearl

Taletrectinib is a next-generation, brain-penetrant ROS1 TKI with very high response rates in TKI-naive ROS1-positive NSCLC (cORR ~85-90%) and meaningful activity after prior ROS1 TKIs, including against the G2032R resistance mutation. Its CNS penetration addresses the frequent brain metastases in this population. Dose on an empty stomach and monitor LFTs - hepatotoxicity and GI effects are the most common issues.
