Mechanism of Action

Tamoxifen is a selective estrogen receptor modulator (SERM) that competitively binds the estrogen receptor (ER) in a tissue-selective manner. In breast tissue, the tamoxifen–ER complex recruits transcriptional co-repressors rather than co-activators, blocking estrogen-responsive gene transcription and suppressing tumor cell proliferation. CYP2D6 pharmacogenomics is one of the most clinically consequential drug–gene interactions in oncology: co-administration of paroxetine with tamoxifen has been estimated in multiple pharmacoepidemiologic cohort studies to increase breast cancer mortality risk by approximately 25%, due to near-complete abolition of endoxifen generation.
