Mechanism of Action
Oral imidazotetrazine prodrug that spontaneously converts to MTIC (methyltriazen-1-yl-imidazole-4-carboxamide) at physiological pH. MTIC methylates guanine at the O6 position, causing DNA mismatch, strand breaks, and apoptosis. Unlike streptozocin, temozolomide does not rely on GLUT2-mediated selective uptake into islet-derived cells — it distributes broadly to all tissues with good oral bioavailability and CNS penetration. FDA Indications
- Newly diagnosed glioblastoma multiforme (GBM) — concomitant with and following radiotherapy (Stupp protocol)
- Refractory anaplastic astrocytoma
- NOTE: Use in GI/carcinoid NETs is entirely off-label; temozolomide carries no FDA approval for neuroendocrine tumors of any type

- Nausea and vomiting (moderate emetic risk; 5-day schedule)
- Fatigue
- Lymphopenia (universal; may be severe)
- Thrombocytopenia (nadir days 22–26)
- Neutropenia
- Constipation
- Headache
- Anorexia

Clinical Pearl
The ECOG E1281 phase III trial randomized patients with metastatic carcinoid tumors to streptozocin + doxorubicin vs. streptozocin + 5-FU, and demonstrated that traditional streptozocin-based regimens offer modest objective response rates in carcinoids (4–16%) — substantially lower than in pNETs — reflecting the lower GLUT2 expression and chemotherapy-resistant biology of midgut-origin tumors.
