Tretinoin (all-trans retinoic acid; ATRA) is a biologically active vitamin A derivative that directly targets the PML-RARα fusion oncoprotein driving acute promyelocytic leukemia (APL). The t(15;17) translocation generates PML-RARα, which recruits co-repressor complexes (NCoR, SMRT, HDAC) to RARα target gene promoters, arresting myeloid differentiation at the promyelocytic stage. ATRA pharmacokinetic tolerance develops after 1–2 weeks of continuous treatment due to CYP3A4 auto-induction and upregulation of cellular retinoic acid-binding proteins (CRABPs), reducing plasma ATRA levels 5–10-fold.
