Mechanism of Action
Tretinoin (all-trans retinoic acid; ATRA) is a biologically active vitamin A derivative that directly targets the PML-RARα fusion oncoprotein driving acute promyelocytic leukemia (APL). The t(15;17) translocation generates PML-RARα, which recruits co-repressor complexes (NCoR, SMRT, HDAC) to RARα target gene promoters, arresting myeloid differentiation at the promyelocytic stage. FDA Indications
- Induction of remission in adults with APL (FAB M3) characterized by t(15;17) translocation and/or PML-RARα gene expression — used in combination with arsenic trioxide for low/intermediate-risk APL, or with anthracycline-based chemotherapy for high-risk APL (FDA approved 1995)

ATRA pharmacokinetic tolerance develops after 1–2 weeks of continuous treatment due to CYP3A4 auto-induction and upregulation of cellular retinoic acid-binding proteins (CRABPs), reducing plasma ATRA levels 5–10-fold. This phenomenon supports intermittent dosing schedules used in consolidation and maintenance.
