Mechanism of Action

Third-generation nitrogen-containing bisphosphonate with the highest potency of the bisphosphonate class. Binds with very high affinity to hydroxyapatite mineral surfaces in bone, concentrating at sites of active osteoclast-mediated bone resorption. Internalized by osteoclasts during bone resorption, where it inhibits farnesyl pyrophosphate synthase (FPPS), a key enzyme in the mevalonate (cholesterol biosynthesis) pathway. ONJ is the most feared long-term complication — prevention is far easier than treatment. Always obtain a dental consultation and complete all necessary dental work BEFORE starting zoledronic acid; elective extractions, implants, and other invasive procedures should be deferred while on therapy. The acute-phase reaction to the first infusion (flu-like symptoms, bone pain) is largely preventable with prophylactic acetaminophen.
