CALGB 140503 — Lobar or Sublobar Resection for Peripheral Stage IA Non-Small-Cell Lung Cancer

Phase III · 2023 · Non-Small Cell Lung Cancer

Design

Multicenter, international, randomized phase III non-inferiority trial comparing sublobar resection (wedge or segmentectomy) with lobectomy in peripheral stage IA NSCLC <=2 cm with pathologically confirmed node-negative disease.

Population

697 patients with peripheral clinical T1aN0 NSCLC <=2 cm and confirmed negative hilar/mediastinal nodes, randomized intraoperatively.

Primary Endpoint

Disease-free survival

Key Result

Sublobar resection was non-inferior to lobectomy for disease-free survival (HR 1.01; 90% CI, 0.83-1.24), with similar 5-year DFS (63.6% sublobar vs 64.1% lobectomy) and 5-year overall survival (80.3% vs 78.9%).

Guideline Impact

Endorsed sublobar resection as standard for small peripheral NSCLC
