NRG-BR007 / DEBRA — De-escalation of adjuvant radiation in low-risk, hormone-sensitive early breast cancer

Population

Stage I, HER2-negative, hormone-receptor-positive breast cancer with an Oncotype recurrence score ≤18 after lumpectomy.

Primary Endpoint

Ipsilateral breast recurrence (non-inferiority)

Rationale

Radiation after lumpectomy is standard, but genomically low-risk tumors may not need it. DEBRA tests whether endocrine therapy alone controls local recurrence, allowing radiation to be omitted.

What It Could Change

Could let genomically low-risk early breast cancer patients safely skip radiotherapy.
