Untreated metastatic NSCLC with PD-L1 TPS ≥50% and no actionable genomic alterations.

Rationale

Patients with high-PD-L1 NSCLC can receive pembrolizumab alone, but most eventually progress. TROPION-Lung08 tests whether adding the TROP2-directed antibody-drug conjugate datopotamab deruxtecan up front deepens responses without the toxicity of platinum chemotherapy.

What It Could Change

Could establish a chemotherapy-free but intensified first-line option for the high-PD-L1 subgroup.
