Mechanism of Action

CELMoD (cereblon E3 ligase modulator) that binds cereblon (CRBN) — the substrate receptor of the CRL4^CRBN E3 ubiquitin ligase — with markedly higher affinity than IMiDs (thalidomide, lenalidomide, pomalidomide). This enhanced binding drives deeper and more rapid ubiquitination and proteasomal degradation of the transcription factors Ikaros (IKZF1) and Aiolos (IKZF3), which are essential survival and proliferation factors in multiple myeloma plasma cells. The higher cereblon affinity allows iberdomide to overcome IMiD resistance mechanisms including CRBN downregulation and IKZF3 mutations.
