Magrolimab — Investigational Agent

Mechanism of Action
First-in-class humanized IgG4 monoclonal antibody against CD47, the "don't eat me" signal overexpressed by many tumors. CD47 binds SIRPα on macrophages to suppress phagocytosis; magrolimab blocks the CD47–SIRPα axis, restoring macrophage-mediated phagocytosis of malignant cells. It synergizes with agents that increase pro-phagocytic "eat me" signals (e.g. calreticulin exposure induced by azacitidine, or antibody Fc engagement with rituximab). A priming/maintenance dose strategy mitigates on-target anemia from clearance of aged CD47-expressing red blood cells.
Investigational Indications
- Higher-risk myelodysplastic syndromes (MDS) — in combination with azacitidine
- Acute myeloid leukemia (AML), including TP53-mutant disease — in combination with azacitidine (development scaled back after phase 3 futility/safety signals)
- Relapsed/refractory non-Hodgkin lymphoma (DLBCL, follicular) — in combination with rituximab
- Solid tumors (investigational, earlier-phase combinations)
