Mechanism of Action
Oral, selective small-molecule inhibitor of PIM1 kinase (with activity against PIM2/3). PIM kinases are serine/threonine kinases downstream of JAK-STAT signaling that promote cell survival, proliferation, and fibrosis-driving cytokine production. In myelofibrosis, PIM1 is upregulated and cooperates with JAK2 signaling; inhibiting it reduces pro-inflammatory and pro-fibrotic cytokines (including TGF-β), aiming to improve cytopenias, reduce spleen size, and modify the underlying bone-marrow fibrosis rather than only palliate symptoms.
Investigational Indications
- Relapsed/refractory myelofibrosis (monotherapy)
- Myelofibrosis in combination with JAK inhibitors (ruxolitinib or momelotinib)
- Higher-risk myelodysplastic syndromes (exploratory)
