Mechanism of Action
Axicabtagene ciloleucel (axi-cel) is an autologous CD19-directed CAR-T cell therapy utilizing a retroviral vector to express a second-generation CAR with an anti-CD19 FMC63 scFv, CD28 transmembrane and costimulatory domain, and CD3ζ signaling domain. The CD28 costimulatory domain drives rapid and robust early expansion compared to 4-1BB-based constructs. Infused CAR-T cells bind CD19 on malignant B cells leading to activation, proliferation, and targeted cytolysis. FDA Indications
- Relapsed or refractory large B-cell lymphoma (DLBCL, PMBCL, high-grade B-cell lymphoma, DLBCL from follicular lymphoma) after two or more lines of systemic therapy in adults (FDA approved October 2017)
- Relapsed or refractory follicular lymphoma (FL) after two or more lines of systemic therapy in adults (FDA approved September 2022)
- Relapsed or refractory large B-cell lymphoma in adults who have not responded to or relapsed within 12 months of first-line chemoimmunotherapy (second-line setting; FDA approved April 2022)

Clinical Pearl
CD28 costimulatory domain results in faster and higher peak expansion than 4-1BB products, contributing to higher early response rates but also historically higher rates of severe ICANS. Early corticosteroid use for grade 2 ICANS is recommended per ASTCT guidelines. Axi-cel also has a second-line LBCL indication, making disease timeline documentation critical.
