Mechanism of Action

Bleomycin is a glycopeptide antitumor antibiotic that exerts its cytotoxic effect primarily through DNA strand scission. It chelates ferrous iron (Fe²⁺) and reacts with molecular oxygen to generate reactive oxygen species — principally superoxide and hydroxyl radicals — that cleave both single-stranded and double-stranded DNA at specific G-C and G-T sequences. Because bleomycin's cytotoxicity is not dependent on active DNA synthesis, it is cell-cycle active principally in the G2 and M phases. Bleomycin's role in ABVD for Hodgkin lymphoma is increasingly questioned — the RATHL trial demonstrated that PET-negative patients after 2 cycles of ABVD could safely omit bleomycin (switching to AVD) without compromising disease control, significantly reducing pulmonary toxicity.
