Mechanism of Action

Dacarbazine is an alkylating triazene prodrug that requires metabolic activation via N-demethylation by hepatic CYP1A2 to generate the active monomethyl triazeno metabolite, MTIC (5-(3-methyltriazen-1-yl)imidazole-4-carboxamide). MTIC spontaneously decomposes to release a methyldiazonium ion that methylates DNA predominantly at the O⁶ and N⁷ positions of guanine, forming DNA adducts that cause cross-links and strand breaks, ultimately inhibiting DNA replication and transcription. Dacarbazine was the historical standard single-agent therapy for metastatic melanoma (response rate ~15–20%), but has been largely supplanted by BRAF/MEK inhibitors (for BRAF V600-mutated disease) and checkpoint inhibitors (ipilimumab, nivolumab, pembrolizumab). It retains relevance as the 'D' component of ABVD for Hodgkin lymphoma.
