Mechanism of Action

Melphalan is a bifunctional phenylalanine mustard alkylating agent that forms highly reactive carbonium ion intermediates via intramolecular cyclization. These intermediates alkylate the N7 position of guanine in DNA, creating interstrand and intrastrand DNA crosslinks that prevent DNA replication and transcription. The drug is cell-cycle nonspecific, acting on both dividing and resting cells. Its structural similarity to phenylalanine allows active transport into cells via amino acid transport systems, contributing to its activity in amino acid-avid tumor cells.

High-dose melphalan 200 mg/m² (HDM200) followed by ASCT remains the standard of care consolidation for transplant-eligible newly diagnosed multiple myeloma, with the Evomela (propylene glycol-free) formulation preferred due to reduced injection-site and hypersensitivity reactions.
