Mechanism of Action

Ciltacabtagene autoleucel (cilta-cel) is an autologous BCMA-directed CAR-T cell therapy featuring a unique dual-VHH (nanobody) design with two camelid-derived single-domain anti-BCMA antibody fragments arranged in tandem on the CAR construct, providing bivalent BCMA binding with potentially enhanced avidity. Cilta-cel has a distinctive delayed neurotoxicity profile including movement and neurocognitive symptoms (Parkinsonism-like features) that can emerge weeks to months post-infusion — distinct from classic early ICANS. Oncology teams must educate patients and caregivers to report new neurological symptoms even after hospital discharge. The mandatory 28-day proximity requirement near the certified center must be incorporated into the patient's logistical planning.
