Mechanism of Action

Idecabtagene vicleucel (ide-cel) is an autologous BCMA-directed CAR-T cell product based on the bb2121 lentiviral construct, incorporating a single anti-BCMA scFv fused to 4-1BB costimulatory and CD3ζ signaling domains. BCMA is highly expressed on malignant plasma cells, making it an attractive target in multiple myeloma. Following lymphodepletion and reinfusion, ide-cel T cells expand, traffic to bone marrow, recognize BCMA on plasma cells, and execute perforin/granzyme-mediated cytotoxicity and ADCC-like mechanisms. Ide-cel (bb2121 backbone, single anti-BCMA scFv) and cilta-cel (dual-nanobody BCMA binder) represent distinct BCMA CAR-T architectures with different CRS timelines and severity profiles. Ide-cel CRS tends to peak earlier (Days 1–5) compared to cilta-cel (median Day 7). Ide-cel does not carry the specific cilta-cel warning for delayed movement/neurocognitive symptoms, though ICANS monitoring is still essential.
