Mechanism of Action

Panobinostat is a hydroxamic acid pan-deacetylase (pan-HDAC) inhibitor with potent activity against all class I (HDAC1, 2, 3, 8), class II (HDAC4, 5, 6, 7, 9, 10), and class IV (HDAC11) zinc-dependent deacetylases. By chelating the catalytic zinc ion, panobinostat blocks histone deacetylation, resulting in global histone hyperacetylation (H3, H4), chromatin relaxation, and transcriptional reactivation of silenced tumor-suppressor genes. The PANORAMA-1 trial (Richardson et al., Lancet Oncol 2014) demonstrated that panobinostat + bortezomib + dexamethasone significantly improved median PFS over placebo + bortezomib + dexamethasone (12.0 vs 8.1 months; HR 0.63) in relapsed/refractory MM, with greatest benefit in patients who had received both prior bortezomib and an IMiD (PFS 12.5 vs 7.0 months).
